Questionnaires on Embodia

As with most resources that are part of a consult, you can create a questionnaire just for your individual membership, or if you are the manager of a clinic with a clinic membership, you can create a questionnaire available for everyone in your clinic.

Managing questionnaires for individual memberships

To manage questionnaires that are part of your individual membership, click on Questionnaires under the HEP tab in the top bar.

Managing questionnaires for clinic memberships

If you are the clinic manager, you can manage the questionnaires available to everyone in the clinic by clicking on the Questionnaires > For the clinic account under the HEP tab in the top bar.
