Questionnaires on Embodia

Yes, a patient can upload a file which you can then view. This feature is available for all Tier 3 members on Embodia. You can view our membership tiers and pricing here.

To allow a patient to upload a file, set up a questionnaire by going to HEP > Questionnaires and either 'For my personal account' or 'For my clinic account':

Select Add questionnaire:

Give your questionnaire a name and select the type of questionnaire - the file upload function works for any type of questionnaire:

Click on New item > New question:

Write out the question/statement such as 'Please upload your imaging results' and then select the Question type to be 'File upload':

You can then share this questionnaire with your patient.

Please note: you can allow your patients to upload one file per question on the questionnaires with a maximum size of 5MB (per file). It allows images and file uploads, but not videos.

 

The Patient Experience

Your patient will be notified about the new questionnaire and they can complete it by logging into their secure Embodia account. This is how a file upload will look to your patient:
